How to refer
Email or call our central intake team with a brief overview of the participant, their location and the supports they're looking for. We'll confirm availability and arrange a meet-and-greet.
My Health Connect is currently undergoing NDIS registration. Service delivery, staffing and pricing are aligned with NDIS Practice Standards and price limits.
