Who we are

Building support that's easier to access, understand and manage.

My Health Connect was created to make disability and community support easier to access, understand and manage. We combine consistent service standards with local teams that understand their communities, currently delivering support across the ACT and selected NSW regions.

My Health Connect is one organisation with one quality framework, one intake team and one participant experience — supported by small local teams that participants and families come to know and trust.

Our four-layer operating model keeps consistency high and local relationships strong. Quality and governance sit alongside local knowledge and individual support planning — never in place of them.

    State-based service management aligned to each state's regulatory landscape — with clear accountability for local performance, workforce and stakeholder relationships.

    Within each state, we operate in defined regional service areas so that participants receive support from workers who know their community.

Central referral process

One point of contact for enquiries and referrals — a warm, consistent handover to the local delivery team.
