Our privacy commitments
My Health Connect handles personal information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. We only collect the information we need to provide safe, high-quality supports and to meet our obligations under the NDIS.
What we collect
- Contact details (name, phone, email, address)
- NDIS participant number (where applicable, with consent)
- Support-planning information relevant to the service being provided
- Health and safety information relevant to safe service delivery
- Records of interactions, incidents and complaints
How we use it
Personal information is used to deliver services, coordinate with other providers with your consent, meet regulatory reporting requirements, and improve the quality of our supports. We never sell personal information.
How we protect it
Access to personal information is limited to those who need it. Information is stored in access-controlled systems, backed up, and governed by our national privacy and security policies. Workers complete privacy training as part of onboarding.
Online forms
When you submit a form on this website, your answers are sent over an encrypted connection to Web3Forms, a third-party form service, which emails them to us. Web3Forms processes the submission on our behalf; we don't store form answers in your browser. Please don't include more sensitive detail than you need to — you can always call or email us instead.
Your rights
You can ask what personal information we hold about you, request a correction, or make a privacy complaint. Contact us at support@myhealthconnect.com.au. You may also contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
A comprehensive Privacy Policy document will be published as My Health Connect's registration is finalised. Until then, this summary reflects our current practices.
