Why My Health Connect

Nationally designed. Locally delivered.

One national quality framework, delivered by small local teams. These commitments shape every conversation, every visit and every service agreement.

  • Consistent teams

Small, well-supported local teams who get to know each participant and family over time.

  • Participant choice and control

Support shaped around each participant's goals, choices and daily life — never the other way around.

  • Respect and dignity

Plain-language conversations, clear expectations and a commitment to treating people the way we'd want to be treated.

  • Safe service delivery

NDIS Worker Screening, national quality framework, incident-management processes and safeguarding oversight.

  • Privacy

We only collect what we need to respond, and we don't share information without consent.

  • Feedback and complaints

Every participant has clear ways to raise a concern or complaint — and to have it acted on.

How it works

A simple, respectful referral pathway

Whether you're a participant, family member or support coordinator, our process is clear and easy to follow.

  1. 1

    Enquire

Contact us by phone or email. Tell us a little about the participant, their goals and what supports they're looking for.

  2. 2

    Match

We check availability in the participant's location, discuss options and confirm what we can offer.

  3. 3

    Onboard

Service agreement, meet-and-greet with the support team, and a plan tailored to the participant's day-to-day life.

  4. 4

    Begin support

Support commences with a consistent team, regular check-ins, and clear pathways for feedback or change.
