One-to-one and small-group support to help participants take part in community, social, cultural and recreational activities of their choice. Focused on genuine relationships and gradual capacity-building.

How to refer
Contact our central intake team to discuss goals, preferred activities and preferred days. Our local team will meet the participant before support begins.
My Health Connect is currently undergoing NDIS registration. Service delivery, staffing and pricing are aligned with NDIS Practice Standards and price limits.
