Where does My Health Connect currently operate?
We currently deliver services in the Australian Capital Territory and selected New South Wales regions. Support coordination is also available more widely: it's delivered by phone and video, so it's available to participants anywhere in Australia. Other states and territories are not currently open for referrals; register your interest and we'll be in touch as that changes.
Are you a registered NDIS provider?
My Health Connect is currently undergoing NDIS registration. We'll update this page when registration is finalised. In the meantime, our team can explain how this affects your plan type and the supports we can provide.
How do referrals work?
Contact us by email or phone with a short overview of the participant, their goals and location. Our central intake team confirms availability in the participant's area, discusses options, and — if we can support — arranges a service agreement and meet-and-greet with the local team.
Will I be able to choose my supports and workers?
Yes. Choice and control sit at the centre of the NDIS and of our service model. We work with participants to shape supports around their goals, preferences and daily life, and we welcome feedback and changes over time.
How do you keep quality consistent as you expand?
One national quality framework — with consistent policies, incident management, worker onboarding and complaints processes — combined with local service-delivery teams that understand their communities. See our National Quality Framework page for detail.
When will you be available in my state?
Victoria, Queensland, South Australia, Western Australia, Tasmania and the Northern Territory are planned but not currently open. Register your interest via the contact page and we'll let you know when we begin taking referrals near you.
